Part of the Proceeds from the photo shoot will be donated back to your organization!!

Everblades Hockey - Photo Order Form

To make things run smoothly on photo day, here are a few tips:

~ Please report promptly for your team's scheduled photo time

~ Please be sure to have your child's order form filled out

~ Please have a check, money order, or cash attached to the order form

~ If you have any questions - we will have an Information Station located near the photography crew, please refer all questions to our staff at the Information Station - this will allow our photographers to continue processing the players' photos

~ Parents - please let our staff do the work (getting the child's attention, placement of children, & etc...)
